The Rules: You are solely responsible for the equipment you check out. You MUST return it by 8:30 AM the next school day. Failure to properly turn in your equipment will result in suspension of check out privileges. Equipment not returned will be reported as stolen. If you have an equipment malfunction, you must report it immediately upon return. Equipment will be checked out only through submission of this form. Equipment check out is ONLY from 3:30 PM to 8:30 AM the NEXT school day. WARNING: Failure to turn in equipment on time WILL result in detention, possibly ISS!
